Overview

Preservation is the most sophisticated form of beauty.

Charleston goes to extended lengths to restore historic buildings. They repoint, repaint in the same colors and pass them on — and the result is a city that reads as permanent rather than as new. That instinct is unusual in America, and it is the closest thing we have found to a philosophy of aging well.

Applied to a face it means something specific. We are not interested in producing a version of you that did not previously exist. We are interested in the far harder task of holding what is already there: the way your brow moves, the proportion of your midface, the quality of your skin before twenty Charleston summers have their say.

Which requires restraint, and restraint requires judgement. It is much easier to inject generously than to know when to stop, and easier still to sell a patient the treatment she asked for than the treatment she needs. Every protocol on this site is written to make the harder choice the default.

Every first treatment is dosed conservatively and reviewed at two weeks. One extra visit beats three months of looking treated.

Assessment before treatment

Photographs, magnification and a plan before a product is named. Most patients need less than they arrived expecting.

A five-year horizon changes every decision — which treatment, what season, and whether to treat at all this month.

Realistic treatment outcomes

Wrong tool, wrong season, wrong age, or a case for a surgeon. We would rather lose the appointment.

A Lowcountry practice.

Protocols borrowed from a dry climate do not survive here. Ours are written for year-round UV, eight months of heat, and salt air.

What does "preservation" actually mean here?

Treating to hold what you have rather than to change it. Practically that means conservative dosing, treatments planned over years rather than appointments, and a bias toward doing less than a patient asks for and reviewing in two weeks.

Will people be able to tell I have had something done?

That is the standard we are judged by. Our aim is that friends tell you that you look well-rested and cannot say why. If a result announces itself, we consider that a failure of judgement rather than a matter of taste.

Is a conservative approach more expensive over time?

Usually less. Under-treating and refining costs one extra visit; over-treating costs months of waiting for a result to wear off, and in filler it can cost a dissolving appointment and a correction.
